What is a risk matrix? The risk matrix, often also referred to as a risk diagram, is a tool used to help visualize the risk in order to help the assessor determine the severity potential of the risk and its potential to cause an undesirable effect or consequence.

Although there are various approaches for assessing risk through a risk matrix, it is important for a company to use a matrix that can meet their needs while also ensure risk can and will be assessed consistently through the matrix in order to lay the foundation for continual improvement opportunities within the functioning management system.
